The Top 10 Things That Go Wrong When an senior Comes Home from the Hospital or rehab (And How to safeguard against Them)

Coming home from the hospital or rehab is supposed to feel like a relief. Instead, for many people it’s where the real stress begins.

Here are the top 10 problems after discharge and what to do instead

    1. The Medications Have Changed

Discharge instructions are often confusing and the new medication list doesn't match what’s at home.

Fix It:

Do a full medication inventory within 24 hours

Use one pharmacy to fill all medications

Create a simple daily medication chart or pill box

    2. The Care Plan is Confusing

Patients leave with paperwork but not clarity.

Fix It:
Before discharge ask:

“What are the things that would cause a readmission?”
“What should we do if something changes?”

    3. The Home Isn’t Set Up for Safety

Falls happen fast, especially while a patient is still recuperating.

Fix It:

Remove rugs

Add grab bars

Have assistive devices ready

Ensure clear walking paths

Set up a main-floor living space if possible

    4. Follow-Up Appointments Are Missed or Aren't Scheduled

This leads to gaps in care and missed warning signs.

Fix It:
Schedule all follow-ups before leaving the facility.

    5. Pain Is Either Over-Treated or Under-Treated

Recovery often means less pain and changes to medication regime over time.

Fix It:
Use a structured medication schedule.  Speak with the primary care provider about all medications.

    6. Nutrition Falls Apart

Appetite drops, but healing requires fuel.

Fix It:

    Prepare simple, protein-rich meals
    Use supplements and snacks
    Remember to hydrate

    7. Family Caregivers Burn Out Quickly

    Most underestimate the workload.

Fix It:
Plan shifts/breaks, even if informal. No one should do this alone.

    8. Mobility Declines Instead of Improves

If you don't use it, you will lose it. Without movement, recovery stalls.

Fix It:
Follow therapy instructions daily—even lightly.

Arrange for home therapy or outpatient therapy.

    9. Warning Signals Are Missed

Small changes are often a sign of bigger problems.

Fix It:
Watch for:

Increased confusion

Increased pain

Swelling

Shortness of breath

Sudden fatigue

    10. Not having anyone "in charge"

When everyone assumes the patient or someone else is handling things, things fall apart.

Fix It:
Assign one person as central care coordinator (someone other than the patient, if possible).

       Bottom Line

Discharge home isn’t the end of Recovery.
